Configuring lead forms for Meta (Facebook) campaigns

Create lead forms in Blueprints and easily apply them across multiple accounts.

Instant forms (or what we call lead forms in Fluency) for Meta lead campaigns are simple to create in Blueprints. Once set up, instant forms can be applied to multiple accounts from a single Blueprint.


Creating a lead form template

  1. Hover over Automate (rocket icon) in the left menu, and select Lead Form Templates.

  2. Click + Add New Template—you may need to scroll down to find it.

  3. Fill out the Lead Form Blueprint Name and Blueprint Vertical fields.

  4. Complete the additional fields, found in the dropdowns:

    • Intro: Provide an Intro Headline and Intro Content. (Optional) Enter a Cover Photo URL to use in this lead form. When left blank, Facebook will use a photo from the attached ad.

    • Questions: Include the fields you'd like a user to fill out when they submit a form (e.g., First Name, Email, Phone, etc.). Click + Add Question to add more fields. Select Custom Text Input to customize your own questions. (Optional) Enter a Question Headline to customize the headline on the top of questions.

    • Custom Disclaimer (Optional): Add if applicable.

    • Privacy Policy (Required): Provide a privacy policy URL.

    • Thank You: Enter the message you want users to see after they submit their information.

    • Options

      • Form Type: Select "More Volume" or "Higher Intent."

      • Form Sharing: Select "Restricted" or "Open."

      • Require Leads to Verify Their Phone Number With a One-Time Passcode: Select “No” or “Yes.” Note: This will not apply unless the form type “Higher Intent" is selected.

      • URL Parameters & Promoted Page Id: Inputting values here allows you to define the promoted page ID within the lead form using a tag.

  5. Once you've completed all the necessary steps, click Save in the top-right.

Note: You can enter URLs directly or by using a tag (e.g., [account.domain]/privacy-policy). Use the tag pane to the right to find the tag you need and copy/paste it into the appropriate field.


Adding a lead form template to a lead ad

Once you've created a lead form template, you'll need to assign it to a lead ad.

  1. Hover over Automate (rocket icon) in the left menu, and select Blueprint Editor.

  2. Select a Blueprint from the list and then a Facebook campaign.

  3. In the Basic section of the Blueprint's information, select Leads in the Goal field.

  4. Click Save Changes.

  5. Click the + New Ad Group button toward the bottom of the window—you may need to scroll down. For the following fields, select:

    • Conversion Location (Destination Types): Select Instant Forms and Messenger.

    • Performance goal: Select Lead Generation.

  6. Click Save Changes.

  7. In the new ad group, select the Creatives dropdown, and click + New Creative.

  8. In the tags pane on the right, select the Shared tab.

  9. Click the + next to lead-form-templates.

  10. Click on the lead form you want to use to copy it to your clipboard.

  11. Paste it into the field Lead Form Template for your creative.

  12. (Optional) Enter the partner created lead form name under Lead Form Name.

    Note: Enter the name exactly as it appears at the partner or use wildcards. To use wildcards, enter a portion of the Meta lead form name with an asterisk at the end (e.g., The Meta lead form name is “Yoga Class March 2026,” entering Yoga Class* will match to any lead form that started with the words Yoga Class). If your lead form at the partner includes an asterisk in the name, enter a backslash to use the asterisk as an actual character (e.g., Yoga Class\*).

  13. Click Save Changes.
